ABDOMINAL FUSIFORM SARCOMA AFTER THOROTRAST (in German)

In a 67-year old man, an intra-abdominal fusiform cell sarcoma developed twenty years after thorotrast application by arteriography of the right leg. Thorotrast deposition was detected by x rays in the liver and spleen. The sarcoma had affected the omentum, the mesentery, and the intestinal fluid, penetrated the pancreas, and formed a retroperitoneal tumor with involvement of the left adrenal gland and fine and coarse nodular depositions in the liver. A significant thorotrast storage was detected microscopically in the liver and to a small extent in the kidney. Thorotrast was detectable at the perimeter of the tumor site in the liver. Pigmentation inclusions in the tumor cells could not be detected. In critical evaluation of these findings, the present case partially fulfills the hypotheses for the recognition of a tumor originating from thorotrast effects. The latency time of 20 years corresponds to the assumption made by Bauer on thorotrast sarcomas. The malignant character of the new formation was proved. Other factors for the tumor development without a relationship to the thorotrast application was not established. The findings are therefore positive for the question of the relationship between thorotrast application and expected tumor development. In spite of the detection of thorotrast precipitations in the perimeter of the tumor site in the liver, it is not proved, in the complete localization of the tumor, that the position of the contrast medium precipitation is connected with the site of the tumor origin. For a positive relationship, the data must be evaluated with the reservation that the sarcoma first appeared in a patient at the advanced age of 67. (tr-auth)
