CAVERNOUS ANGIOPLASTIC ENDOTHELIAL SARCOMA OF THE LIVER AND HEPATIC THOROTRASTOSIS
The case history is described of a 31-yr-old man who died of sarcoma of the liver 17 yr after diagnostic arteriography employing Thorotrast. Evidence is provided that development of the tumor can be attributed to Thorotrast. The tumor was not metastatic but occupied a large part of the liver. ThO/sub 2/ could be demonstrated in fibrotic portions of the tumor, throughout the liver, and in reticuloendothelial cells in the spleen, lymph nodes, and other tissues. Para-arterial deposits of ThO/sub 2/ were also seen and the condition was considered to reflect generalized thorotrastosis, including thorotrastic cirrhosis. Clinically, the tumor appeared to develop over a 4-yr period, causing irreparable hepatic insufficiency. The tumor was of mesenchymal nature with an angiomatous structure. It was formed from endothelial elements of liver sinusoidal capillaries. Eleven other cases of thorotrastomas reported in the literature are reviewed. The average time between introduction of Thorotrast to detection of the tumor is 13 yr. The rare nature of the neoplasia, its constant association with ThO/sub 2/, and the known carcinogenic action of ionizing radiation appear to implicate Thorotrast in the development of these tumors.
