FAILURES AND HAZARDS IN OPERATIONS FOR DIVERTING THE URINARY TRACT IN WOMEN WITH IRRADIATED GENITAL CARCINOMA (in German)

Among 1530 women submitted to radiotherapy for pelvic cancer, 49 (3.None2%) developed serious urinary tract complications. The complications in these 49 cases could be broken down as follows: severe radiation cystitis 11, cystitis and ureteral stenosis with urinary stasis 6, bilateral ureteral stenosis and stasis 18, unilateral stenosis with stasis 7, and ureteral-, rectal-, or vaginal-vesical fistulas 7. Of 15 patients treated conservatively with bladder instillations, analgesics, and prednisone, eight were improved, six unimproved, and one died. Of 17 treated surgically by unilateral or bilateral nephrostomy, 7 were improved and 10 died. Nephrectomy led to improvement in each of the five cases so treated, and plastic surgery is only three of six cases, the other three dying. Successful repair of radiation-injured ureters in several cases is described. It is concluded that early diagnosis and treatment of urinary tract injuries following radiotherapy are essential if successful outcome is to be obtained. (BBB)
