ACUTE RADIATION FIBROSIS OF THE LUNG (in German)

Symptoms of radiation fibrosis of the lung appear from one to two months after termination o; the radiation treatment. This latency period varies with the intensity of the radiation administered and the general physical condition of the patient. Anatomically the pathologic signs of fibrosis are destruction of the alveolar epithelium, degeneration and metaplasia of the bronchial epithelium, damage to the bronchial tubes, peribronchial, perivascular, and interstitial inflammatory processes, as well as cicatrization from broncho-pneumonia and atalectasia. The clinical evidence for these changes are dyspnea, cough (productive or nonproductive), elevated temperatures, and increased erythrocyte sedimentation rate. Pain with breathing indicates involvement of the pleura. Increased pulmonary circulation resistance may lead to cor pulmonale. With extensive fibrosis, functional damage to the lung parenchyma with hyalinization and calcification may impair circulation enough to be fatal. Increased fibrosis reduces long elasticity and vital capacity. During the latent period before appearance of the symptoms the air capacity of the affected area is increased, and radiologically there is a fine shadowing of the area, that has an appearance of pleuritis or pneumonitis, and this intensifies as fibrosis progresses. The changes are restricted to these areas. Differential diagnosis is required and it is necessary to differentiate between interstitial diffuse carcinomas, and cicatrizing inflammations that often accompany fibrosis. Vitamin K, often used to sensitize tumors to radiation, tends to promote fibrosis. Treatment of radiation fibrosis is essentially symptomatic, and therapy is difficult. Prednisone has some effect, but antibiotics are usually ineffective. (BBB)
