Brachytherapy with Cf-252 in USSR: Head and neck, GYN and other tumors

The USSR program in /sup 252/Cf neutron brachytherapy has been conducted in 5 centers which have conducted clinical trials on --721 patients through early 1985. The majority of patients had head and neck tumors (61%), uterine cervix and uterus (27%) and other sites (skin, soft tissues, etc. - 12%). In 83% the histology was squamous cell carcinoma, 14% adenocarcinoma and 3% melanoma, sarcoma, etc. More rapid tumor clearance followed /sup 252/Cf treatment than after /sup 60/Co irradiation for head and neck, and cervix cancers and no resistant histological types were found. Complete regression of recurrent tumors was observed in 81/93 (87%) of patients treated. In 142 head and neck tumors of the tongue, floor of mouth and oropharynx, early vs. delayed /sup 252/Cf implants were compared against conventional /sup 60/Co therapy. Delayed neutron therapy was somewhat worse than conventional photon therapy. For T2 tongue carcinoma, complete regression was obtained in 96% and 2 year survival in 92% with early implants vs. 53% for delayed neutron therapy and 75% for /sup 60/Co. For T2 floor of mouth cancer complete regression was obtained in 100% and 2 year survivals were 86% for early /sup 252/Cf implants vs. 46% for delayed /sup 252/Cf and 66% for conventional therapy. For 65 advanced cervix cancers treated with delayed /sup 252/Cf therapy 62% survived 2-5 years and 60 control patients treated with conventional radiation 55% survived 2-5 years. For patients with corpus adenocarcinomas the survival in 37 patients was 94% vs. 73% in 30 control patients. Complication rates in each site were of the same degree and severity as with photon radiotherapy.
