Effect of nifedipine on right ventricular ejection fraction during exercise

First pass Kr-81m studies for right ventricular ejection fraction (RVEF) measurements allow multiple repeat studies within 1-2 minutes of each other, because of Kr's 13 sec half life and clearance by the lungs. In 12 normal volunteers, aged 22-35, multiple measurements of RVEF at rest, 25,50 and 100 w-s exercise, 3 and 5 min post exercise showed an average pooled RVEF of 65 +- 5, 70.2 +- 6.5, 72.7 +- 5.7, 74.5 +-7.7, 71 +- 4.4, 68.3 +- 4.0, respectively. There was good reproducibility at rest and at each exercise stage. The overall coefficient of variation was 4.17 2.55% (rest); 3.87 +- 3.03% (stress). First pass Kr-81m determination of RVEF can demonstrate changes at each stage of exercise, correlates with Tc-99m, and can be used in multiple repeat studies for determining the role of pharmacologic manipulation on RV function. In this study, despite a significant reduction in pulmonary vascular resistance with both 02 and Nifedi-pine, there was no improvement in resting or exercise RVEF.
