Modified fatty acid analog imaging: Correlation of SPECT and clearance kinetics in ischemic-reperfused myocardium

To detect metabolic activity in ischemic-reperfused canine myocardium under conditions of varied myocardial blood flow (MBF), 6 anesthetized close-chested dogs underwent sequential thallium-201 (T1-201; 1-2mCi) and I-123 15-(p-(iodophenyl))-3-methylpentadecanoic acid (I123-MFA; 1.5-5.0mCi) imaging. The left anterior descending coronary artery was occluded for 1 hour, followed by release for 2-3 hours. T1-201 was injected IV and SPECT images recorded. At the conclusion of T1-SPECT, I123-MFA was injected and both sequential images and a second set of SPECT images recorded. Qualitative interpretation of T1-201 and I123-MFA SPECT images revealed I123-MFA uptake in excess of T1-201 uptake in the ischemic border zone in 5/6 dogs. The other dog had no qualitative difference. Initial (0-30 min) and overall (0-120 min) I123-MFA washout rates (w/o; counts/min) and clearance T1/2 values (min) were calculated in 3 dogs from regions-of-interest (ROI) set in the normal posterior wall (N), and the border (B) and central (C) ischemic zones. Thus, I123-MFA demonstrates different kinetic behavior in normal, ischemic and partially infarcted myocardium. Slow I123-MFA clearance from B and C zones may be due to either active metabolism by omega oxidation pathways or passive tracer efflux or both.
