Morphology and function in the experimentally x-irradiated kidney

The kidneys of rabbits were locally irradiated with 4000 to 10000-r doses of 60- or 250-kv x rays, and histologic studies and tests of renal function were conducted from 9 days to 20 months later. A chronic, markedly progressive contracted kidney with severe nephrosis, especially of the distal tubular sections, was produced. The kidney changes were accompanied by a distal tubular syndrome characterized by retention of phenol red, electrolyte disturbances, anemia, hypertension, renal osteopathy, and terminal uremia. After an early stage of swelling, the kidney shrunk to 3/5 of its normal weight by 1 to 1 1/2 months later. Papillary neurosis and pyelonephritis were evident in some cases. Striking was the high radiation resistance of the glomerulus. A distinct difference between the behavior of renal tissue of irradiated rabbit and human kidney was noted. Human kidneys show chiefly glomerular injuries, rabbit predominantly tubular lesions. Vascular and glomerular lesions arose only when the highest radiation dosage was employed or when extraradiation factors, such as hypertension, inflammation, and edema, were implicated. At late periods, fibrosis was the principal feature of the markedly shrunken kidney.
