Pulmonary mechanics and regional pulmonary functions in normal and diseased subjects were analyzed by the newly provided /sup 133/Xe scanning device. In upright normal men, lung volume, ventilation, and perfusion were regionally affected by gravity as already reported by others. The perfusion/volume ratios were increased from the top to the base of the lung, however the ventilation/ volume ratios were kept constant through the lung. Intrathoracic pressure was related with diaphragmatic movement such that it often indicated very positive in the dependent regions at low lung volume in cases of large diaphragmatic movement. It was concluded that diaphragmatic movement was a very important factor determining the distribution of ventilation. The presented /sup 133/Xe scanning device was suggested quite useful in detection of regional distribution in function such as delays of gas clearance, residual volume/total lung capacity in emphysematous patients. Furthermore it was shown that the present scannlng method was also excellent in analysis of regional perfusion which was not shown by /sup 131/I-MAA scanning method in hilar lung cancer. (auth)
