The report supplements Industrial Engineering Study of Hazards Associated With Underground Coal Mine Production. Vol. 1, Analysis of Underground Hazards and Fatal Accidents (PB-207 226) and Vol. 2, Data and Charts (PB-207 227). Except for discussion of behavioral problems, the various fatal accident problem areas were analyzed both in terms of the potential reduction in fatalities and in relation to the implementation cost of the recommendations. The original computerized fatal accident data base helped determine the relative importance of hazardous conditions, and thus substantially assisted the fatality reduction estimates in the problem areas listed. The ability to tabulate fatal accident frequencies for literally any desired combination of factors was the key to comprehensive analysis. The staff reviewed all the face mining equipment, made field observations of human factor problems, consulted with equipment manufacturers, and analyzed in depth all recorded equipment accident reports.
