Biodistribution of monoclonal antibody B72. 3(I-131) in colon cancer patients

Monoclonal antibody (MAb) B72.3 was generated against a carcinoma metastasis and has been shown to bind with a high degree of selectivity to a tumor associated glycoprotein (TAG-72) with a molecular weight of greater than 200,000 dal. The TAC-72 antigen in present on greater than 80% of human colon carcinomas when tested by immunoperoxidase with no significant reactivity to normal adult tissues. MAb B72.3 was radiolabeled with I-131 and administered intravenously to patients with metastatic colon cacinomas. The amount of MAb B72.3 injected was varied from 0.16-3.95 mg (0.8-10mCi of I-131) without significantly affecting the biodistribution and clearance. Positive scans were obtained in 10 of 19 patients. Most of the patients studied were scheduled for surgical procedures 7-14 days post injection of the MAb allowing us to verify the biodistribution of the MAb. Radioactivity per gram was determined and tumor to normal tissue ratios calculated; these ratios varied from 2:1 to 38:1 in the patients studied. In most lesions the tumor to tissue ratios were greater than 5:1 when compared to any of the normal tissues assayed. At the time of surgery the tumors retained up to 0.01% of the injected dose per gram. Autoradiography confirmed the specificity of the uptake of the I-131 MAb in the tumor masses. The specificity of the localization of MAb B72.3 in vivo demonstrates the potential of this antibody as a diagnostic agent and as a therapeutic agent.
