Draft application forms for EPA programs

The U.S. Environmental Protection Agency issues draft application forms to be used for applying for permits under the EPA's proposed consolidated permits program. The forms included here are: a general form for applicants for EPA permits; a form to apply for National Pollutant Discharge Elimination System (NPDES) permits under the Clean Water Act for concentrated animal feeding operations and aquatic animal production facilities; a form for existing industrial dischargers applying for NPDES permits under the Clean Water Act; and a form for hazardous waste management facilities which require hazardous waste storage, treatment, or disposal permits under the Resource Conservation and Recovery Act. Comments must be received by the EPA on or before 9/12/79. Public hearings will be held in conjunction with those for the proposed consolidated permit program.
