I-123 phenylpentadecanoic acid myocardial imaging at rest and exercise in man: A new noninvasive means to detect ischemic heart disease
I-123 phenylpentadecanoic acid (IPPA), a synthetic long chain fatty acid, has shown promise as a myocardial imaging agent. To assess the utility of IPPA myocardial imaging, 13 normal volunteers (NL) and 16 patients (pts) with coronary artery disease (CAD) were studied. IPPA (3-8 mCi) was injected IV a rest and/or peak exercise. Planar images were acquired for 60 mins. post-injection (p.i.) and quantitative segmental analyses were performed. NLs demonstrate uniform initial normalized IPPA uptake at exercise and rest with standard deviations (SD) of +- 5% and +- 8%. Washout rates were also relatively uniform. Following exercise, all pts with CAD demonstrated heterogenous uptake (>+-2 SD). IPPA uptake in coronary distributions with stenoses > 70% (n=17) was increased in 9, decreased in 6, and normal in 2. Early washout was slow in 7 of 9 pts with initial increased IPPA uptake and in 3 of 6 with initial decreased uptake (4.5 +- 7%, p<0.05). IPPA washout rates generally normalized by the 40-60 min. p.i. Pts studied at rest demonstrated more uniform uptake. Infarct (MI) distributions were characterized by decreased IPPA uptake in 9/9 pts post-exercise and 4/5 pts at rest and washout was generally delayed in the corresponding segments. The authors conclude that (l) high quality myocardial images can be obtained in man with IPPA; (2) MIs demonstrate decreased initial IPPA uptake and washout; and (3) pts with significant coronary stenoses frequently demonstrate increased IPPA uptake and delayed washout. Thus, IPPA imaging may provide a ''hot spot'' marker of ischemic myocardium.
