A formalism is developed for studying ballooning modes for a broad range of equilibrium toroidal magnetic configurations with shear, including tokamaks with a cross-sectional area of arbitrary shape, closed confinement systems with a three-dimensional magnetic axis, and various stellarators with helical windings. A condition is derived for the stability of ballooning modes in a simple system with a three-dimensional magnetic axis: a helical column. At a fixed plasma pressure there is a certain value of the shear at which the ballooning modes become less dangerous than perturbations of the Mercier type.
