The role of human and organizational factors (HOF) in the design and operation of offshore platforms

With the realization that human errors are responsible for the majority of offshore accidents a major offshore oil and gas exploration and production (E and P) company in the Gulf of Mexico instituted, in 1990, a Human and Organizational Factors (HOF) program in the design and operation of their new deepwater platforms to reduce operator errors and increase personnel and platform safety. This paper describes some of the specific activities completed in the HOF effort, provides examples of design and operational changes made on the platforms, describes some of the benefits and costs of the HOF programs, and concludes by offering some lessons learned on how to introduce and operate a HOF program in other companies.
