OBSERVATIONS ON THOROTRAST DEPOSITS IN THE AREA OF THE KIDNEY

Hazards resulting from the use of thorotrast are described. The latent period for tumor development in humans is 20 to 30 yr. Two cases of tumorigenesis following intravascular or intravesicular injections of Thorotrast are described. In the first case Thorotrast had been used to study an aortic occlusion; 13 yr later, examination revealed a thorotrastoma in the renal area, near the aorta, which resulted in constriction of the ureter and incipient hydronephrosis. Microscopically, Thorotrast could be demonstrated in the tumor as a dark pigment surrounded by connective tissue. Some of the pigment was present in macrophages and some was extracellular, in the ground substance. In the second case a transvesicular pyelogram had been carried out to investigate a traumatically ruptured kidney 6 yr earlier. Thorotrast had passed through the rupture site and become deposited around the kidney. This had caused peri- and subcapsular fibrosis, which compressed the kidney, resulting in a painful pyelonephritis. The kidney, which was removed with the thorotrastoma, showed almost complete loss of parenchymal tissue. Development of the benign fibromas in these 2 cases could be attributed to the irritant and radiation effects of ThO/ sub 2/ on the tissues. (TCO)
