THE PERITONEAL GRAFT IN CUTANEOUS RADIONECROSIS EXPERIMENTAL STUDIES
Development of nonhealing ulcers in the skin of patients undergoing radiotherapy is discussed, and methods of applying grafts to these lesions are considered. Experiments were conducted in rabbits to explore the possibility of using homologous or heterologous grafts of peritoneum for this purpose. Lesions in the dorsal skin of rabbits were induced by local application of single 20,000- r doses of 60kv x rays. After 2 weeks the lesions were radically excised to remove necrotic tissue and treated with solutions of penicillin and streptomycin. Immediately, peritoneal tissue obtained from other rabbits or from guinea pigs was sutured over the skin lesion. The animals were injected daily with broad- spectrum antibiotics, whereas in the controls the open lesions were topically treated with powdered sulfonamides or physiologic solution containing penicillin and streptomycin. All of the ulcers (2- to 3-cm dia) treated by peritoneal grafts showed complete healing after 40 days. Six days after grafting the wound showed a clear plaque with fibrin deposition and intense development of granulation tissue. Typical epidermis and dermis developed under the graft, including normal hair follicles. Similar results were obtained with homologous and heterologous grafts. It is concluded that the peritoneal graft protected the ulcer from infection and presented a receptive matrix for proliferation of host cells which rapidly covered the wound. (BBB)
