Development of a pulsatile compartmental model of the cardiopulmonary circulation and its application to the analysis of data from quantitative radiocardiography

Recent studies of cardiopulmonary circulation with the Anger gamma camera have demonstrated the possibility and potential of truly quantitative radiocardiography. Simultaneous measurements of the activity-time functions of an injected radioactive tracer in discrete heart-lung regions have been made available, and the proven feasibility of achieving sufficiently high count rates to measure high frequency events in the cardiac cycle demonstrated. The existing compartmental models used to analyze these data suffer from the limitation of being continuous flow approximations to a truly pulsatile system, and as a result are incapable of simulating the fine structure of the cardiac cycle. In this study, the development of a pulsatile compartmental model is detailed, and the response to a simulated right atrial injection presented. The mass balance equations are developed for a six pool representation, accounting for the pulsatile nature of the flow, and for ventricular volume variation during systole and diastole. The equations are solved by digital computer using techniques of numerical integration. The effects of an atrial septal defect and mitral insufficiency are also studied. Procedures for using the model in conjunction with experimental data for purposes of parameter estimation are suggested. (auth)
