EFFECTS OF ROENTGEN IRRADIATION OF THE JAWS ON SOCKET HEALING IN YOUNG RATS

The pathogenesis of radioosteomyelitis was investigated by x irradiating the molar regions of rats, 52 to 60 days old, at single doses of 1725 and 2400 r. The lower left first molars were extracted either 8 days before irradiation or 5 days after. The observed gross pathologic changes included epilation, erythema, and exudation in the irradiated skin area, and oral changes varying from minor lesions of the floor of the mouth to severe radioosteomyelitis of the jaw with occasional orocutaneous fistulae. The frequency of radioosteomyelitis was 5--6 times greater in rats irradiated before extraction than in those irradiated after the extraction. Microscopically, bone and periodontal membrane necrosis in the socket wall was demonstrated shortly after the extraction. Granulation tissue organdizing the clot showed low cellularity, and epithelial proliferation was impaired. Bone resorption was prominent, but areas with bone formation were also seen. The cellular inflammatory response to surface defects and sequestra was absent in some rats whereas pus accumulation was seen in other irradiated rats. Vascular changes consisting of proliferation of endothelial cells with large nuclei were occasionally noted. When radioosteomyelitis was present, it was characterized by a penetrating destruction of the mandible spreading far beyond the socket area and causing extensive sequestration. Apparently, the irradiated tissues had little capacity of limiting or repairing the damage. It is concluded that if irradiation is postponed until the socket becomes sealed off by organization of the clot and epithelial healing, the changes of radioosteomyelitis development are greatly reduced.
