COMPLICATIONS DUE TO RADIATION OF FOREHEAD AND SCALP

The case histories of five patients are presented; three were treated with x rays or radium for many years for small, localized malignant lesions of the forehead. When these patients were admitted to the hospital, their skin had been destroyed and bone exposed with persistent malignancy. Two further patients who received radiation therapy for small malignancies of the scalp developed severe neurologic symptoms. Two of the malignancies were basal cell carcinomas, the 3rd of undisclosed type. One of the basal cell carcinoma cases died of generalized metastases, whereas the other 2 remained cancer-free. Repair of the radiation-induced ulcers by plastic surgery is described in detail, the results being considered successful in each case. It is concluded that wide excision with plastic repair should be the treatment of choice in malignancies of the forehead and scalp. The widest primary excision and plastic repair is never as difficult as the one required after complications due to irradiation, where the skin is severely damaged, bone exposed, secondary infection present, and the tumor has recurred.
