Safety has normally be described and quantified as an absence of events, we suggest that a more effective approach would rather be the presence of capability. When performing risk important activities if is import to both identify and involve the right people to evaluate the level of risk that can be tolerated or accepted followed by a dialogue to consider the critical steps, where we could focus on the error potential of the workers, the system, as well as consider that our conversations must change considering that if we have a 100% chance to fail, do we have appropriate safeguards in place to fail safely.
