Enhanced applications of human reliability assessment methodology

Recent industry experience indicates that accidents involve a combination of human errors and complex equipment faults. Applications of human reliability assessment methodology (HRAM) in advanced scientific risk applications can help engineers identify and prioritize potential human errors in accident scenarios. Once the error contributors to risk are defined and prioritized, the likelihood of error can be reduced through management actions that facilitate plant operator decision making and enhance the means for detecting and correcting or compensating for errors. This process takes advantage of existing individual plant examinations (IPEs) to propose methods for cost-effectively managing error reduction. Building on insights about human error from event classification systerns, cognitive models, and IPE risk models tools can be built to support reduction of potential errors in specific activities. By expanding on models for repetitive tasks, HRAM could address cognitive tasks involving human interactions with computers such as supervisory control, integrated maintenance and testing, and trouble shooting.
