Estimated left ventricular ejection fraction in rest thallium-201 perfusion imaging

The left ventricular shell thickness (t) to cavity diameter (d) ratio, measured in TL-201 myocardial images in the LAO projection was found to be linearly correlated with the left ventricular ejection fraction (LVEF) calculated from gated radionuclide angiography. The thickness of the posterolateral (t/sub p/), apical (t/sub a/), and septal (t/sub s/) walls were measured and the average t/d was calculated. The t/d ratio was calculated in a group of 72 patients who had also undergone gated radionuclide angiography (GRA). A good correlation (r = .82, SEE = 0.09) was found between t/d and the LVEF measured by GRA. This linear relationship allowed the estimate of LVEF from a measured regression equation. The regression method of estimating LVEF from TL-201 images was then validated in a second group of 85 patients who had undergone cardiac catheterization (r = 0.77, SEE = 0.09). This technique of LVEF estimation evidenced satisfactory reproducibility as demonstrated by the good correlation and reasonable mean variability in inter observer, intra observer and intra study comparison. The value of the SEE is comparable to those commonly obtained in GRA. This technique of LVEF estimation is particularly useful in identifying patients with cardiomyopathy and normal coronaries that exhibit normal TL-201 perfusion images.
