U. S. National Transportation Safety Board (NTSB) reports, recommendations, and responses availability. [Evaluation of fillet-welded reinforcement sleeves]
Following the ignition of crude oil that leaked from a fractured 22 in. oil pipeline at a levee crossing at Berwick, Louisiana, on 1/2/80, NTSB has recommended that Texaco Pipeline Co., the line owner, review the practice of using fillet-welded reinforcement sleeves for tie-in welds to determine if these sleeves have caused any pipeline failures in its system; notify the pipeline patrol of the location of such existing sleeves and instruct it to examine the areas around these sleeves for signs of leakage; and evaluate existing procdures for leak detection to make these procedures more effective. The American Petroleum Institute has also been urged to notify its member companies of the Berwick accident and the aforementioned recommendations. These pipeline and other safety recommendation letters and reports of accidents, including the determination of the cause of the explosion, burning, and sinking of the Liberian tanker Seatige.
