Precursors to potential severe core damage accidents: 1984 a status report: Main report and Appendixes A and B

Forty-eight operational events, reported in LERs and occurring at commercial LWRs during 1984 are considered to be precursors to potential severe core damage. These are described along with associated significance estimates, categorization, and subsequent analyses. This study is a continuation of the work presented in earlier volumes in this series, which evaluated the 1969-1981 and 1985 events. The report sequentially discusses (1) the general rationale for this study, (2) the program methods for review and documentation of operational events as precursors, (3) the use of the conditional probability of subsequent severe core-damage estimates to rank precursor events, and (4) initial conclusions from the assessment of 1984 events.
