Common-mode/common-cause failure: a review and a bibliography

The CM/CC failure phenomenon and a bibliography of the literature are presented. Initial philosophical considerations are presented, and interpretations of various definitions are examined. Classes of CM/CC failures are compiled, and the defenses against such failures and their weaknesses are surveyed. Some regulatory considerations and operating experiences are touched upon briefly. Finally, current trends in analytical treatments and reliability analysis methodology for this phenomenon are presented.
