Visualization of Kent bundle position in WPW syndrome using phase analysis

Multigated blood pool scintigraphy was performed in 20 normal subjects and 20 patients with WPW syndrome. Cardiac imaging was performed in left anterior oblique, right anterior oblique, and left lateral views. In 8 patients with WPW syndrome, gated single photon emission computed tomography (SPECT) was also done. Regional ventricular contraction patterns were evaluated with sequential regional phase mapping. In WPW syndrome, abnormal earlier contraction site could detect at affected ventricular base just below the atrium not seen in normal cases. In three cases double bypass tracts were seen. These abnormal contraction sites were disappeared after blocked conduction with procainamide or surgery. SPECT images were the most reliable to verify the exact position of abnormal contraction sites especially at posterior wall. The authors confirm Kent bundle position in WPW syndrome in 19 cases and conclude: multiprojection or SPECT phase image was suitable method to identify exact position of accessory bypass tract noninvasively; routine left anterior oblique view was not useful position for WPW syndrome.
