The review of operating experience by the Office for Analysis and Evaluation of Operational Data (AEOD) indicates that fundamental root causes underlying significant operational events include design deficiencies and human performance problems. Further, the majority of challenges to safety systems originate in nonsafety systems, primarily those associated with the balance of plant. It is also apparent that, to minimize the frequency and significance of reactor transients, an effective operational experience assessment program is necessary at each plant; simulator training involving multiple failures is necessary to assure realism; comprehensive test and maintenance programs are essential in order to have high confidence that equipment will operate routinely and properly in off-normal, perhaps under accident, conditions; and the support and knowledge provided by training, plant procedures, and plant instrumentation is vital for proper operator performance.
