Metallurgical analyses of mechanical equipment failures

Mechanical equipment failures are shown and described as to the cause of failure, the failure mechanism and the steps taken to prevent recurrence. This paper describes several types of failure mechanisms experienced in mechanical equipment analyzed by the author. Their presentation is not meant to cast reflections on the manufacturers, but hopefully to alert designers and users of mechanical equipment to common pitfalls which should be avoided in the design, manufacture, repair and maintenance of the equipment.
