Demonstration of an angiographically hypovascular insulinoma with intraarterial dynamic CT

The importance of preoperative localization of pancreatic islet cell tumors is well established. Some 90% of insulinomas are less than 2 cm in diameter, and their small size makes these tumors difficult to palpate during surgery. Successful preoperative localization of a suspected insulinoma may prove arduous and could include selective arteriography, sonography, computed tomography (CT) and portal venous sampling. Of these methods, arteriography has proved the most accurate and is positive in 75%-80% of cases. The authors describe a case of a hypovascular insulinoma demonstrated by dynamic CT performed with an intraarterial injection after unsuccessful arteriography.
