Insights into generic safety issues noted from licensee operating events

The Generic Safety Issues (GSI) program was an outgrowth of Public Law 29-209 (amendments to the Energy Reorganization Act of 1974), whereby the U.S. Nuclear Regulatory Commission (NRC) was directed to develop a formalized action plan to identify and resolve outstanding reactor safety issues. During the past two decades, several hundred GSIs were identified and investigated by NRC. By the mid-1990s, solutions to many of these issues had been implemented, so that such GSIs were classified as {open_quotes}resolved.{close_quotes} Reviews by the Advisory Committee on Reactor Safeguards (ACRS) of licensee operational events, however, indicate recurrence of a number of resolved GSIs. An investigation into the adequacy of the agency`s GSI resolution process that was therefore initiated is the subject of this paper.
