This special anniversary review article summarizes significant advances and developments that have occurred in the field of radiopharmaceuticals and nuclear medicine. Advances in technetium-99m chemistry include the development of hepatobiliary radiopharmaceuticals, technetium-99m myocardial perfusion agents and renal imaging. In addition, advances made with radiolabeled antibodies and proteins, therapeutic radiopharmaceuticals, and positron emission tomography are discussed. The authors conclude that radiopharmaceutical development has progressed from a random walk through shelves of reagents to an organized plan of drug design, based upon demonstrated structural-activity relationships.
