All of the currently occurring boiler tube failure (BTF) mechanisms in fossil fuel power plant are understood, and permanent solutions are available for all failures except corrosion fatigue. However, the state-of-the-art understanding of BTF mechanisms, root causes, maintenance, NDE and cycle chemistry are not, by themselves, sufficient to reduce the enormous availability loss due to BTF. This paper reports that in a parallel effort to the root cause work, a management approach was developed and introduced in the form of a formalized company-wide BTF prevention program. The first action was to develop two documents: a generic utility BTF prevention philosophy statement which clearly defined how BTF would be approached in a utility, and a five-module training manual. Once these had been developed, the overall approach was demonstrated in 16 utilities.
