Carcinoma of the prostate: 3 or 5 fractions per week

There is little data on the choice of total dose to guide radiation oncologists who wish to use radiotherapy fractionation regimes for patients with carcinoma of the prostate that differ from the five fractions per week schedule. In this manuscript, we present results of a four year prospective trial of 96 patients with localized prostatic carcinoma (Stage A, B, and C) that used three or five fractions per week. The two groups of patients were comparable in regard to main prognostic factors (stage, degree of tumor differentiation, age) and radiation parameters (field size, treatment technique, total time-dose-fractionation (T.D.F.) values). A small field, rotational technique was used with both groups; the three fractions per week patients received 6000 rad in 20 fractions (T.D.F. -113), and the five fractions per week patients usually received 6300 to 7000 rad in 6 to 7 weeks (T.D.F. 98 to 115). The preliminary results show that the two techniques are very similar in terms of control of the tumor and normal tissue effects. In addition, the three fractions per week technique offers a much more convenient therapeutic approach for patient and physician. These results should not be extrapolated to larger fields, other dosage schedules, or other organ systems. Further follow-up is required to assess long term complications.
