BLOOD RADIATION DOSE AFTER $sup 131$I THERAPY OF THYROTOXICOSIS. CALCULATIONS WITH REFERENCE TO LEUKAEMIA

A case of acute leukemia diagnosed 17 months after I/sup 131/ therapy for thyrotoxicosis is reported. In an attempt to assess the relationship between I/sup 131/ therapy and the subsequent developmeni of leukemia the radiation dose to the blood after a single therapeutic dose of I/sup 131/ was measured in l9l thyrotoxic patients. The mean irradiation to the blood was 8 rads per therapeutic dose. The irradiation dose to the blood is related to the amount of l/sup 131/ administered and the fraction passing into the plasma as proteinbound I/sup 131/. A nomogram taking into account these two factors was constructed from the measurements made on the 191 patients, so that the irradiation dose to the blood can be estimated in other patients treated by I/sup 131/ for thyrotoxicosis. The blood irradiation dose for complete treatment of the disease was estimated from the nomogram in 802 patients previously treated with I/sup 131/ . The mean dose was l6.6 rads, but in some patients receiving multiple treatments it was much greater, ranging up to 160 rads. The bone-marrow dose is estimated as 80% of the blood dose. Consideration of these results suggests that the risk of leukemogenesis is slight and does not contraindicate I/sup 131/ therapy of thyrotoxicosis. Nevertheless it is important to record all cases of leukemia occurring after I/sup 131/ therapy so that the validity of these conclusions may be examined. (auth)
