SOME OBSERVATIONS ON DOSAGE IN IRRADIATION OF PITUITARY ADENOMAS

A survey was made of 23 cases, in which x-ray therapy was given to the sella area in repeated or multiple series totaling 5500 to 9500 r, to determine whether this amount of radiation is safe and whether an optimum dosage can be predicated. The optimum dosage for pituitary adenoma is 4000 r to the sella in a single series. Three cases of malignant pituitary tumors were controlled 6, 9, and 12 yr respectively with doses of 7400to 7900 r in 2 series of treatments. In 4 patients operated on after total dosages of 5400, 5900, 8100, and 9000 r respectively, no radiation change was found in the removed tissues. Necropsy in 2 other patients showed no radiation changes after 7140 and 7200 r. The clinical evidence also indicates that in the dosage range utilized in this group of patients, no untoward developments have occurred as a result of x-ray therapy. Hemorrhage occurred in 1 pituitary adenoma 2.5 yr after an initial dose of 4000 r, but this was not necessarily related to x-ray treatment. Failure to obtain a satisfactory result with 4000 r in benign tumors suggests the presence of a cyst or hemorrhage. In cases of benign tumor, there should be a wait of at least 1 yr after administration of 4000 r to determine results before continuation of x-ray treatment. (H.H.D.)
