Elevated intraocular pressure sometimes follows x-ray therapy directed toward the eye. To investigate the pathogenesis of this disorder, adult guinea pigs were given a single 1400 r dose, either over the whole body or directed toward the iridocorneal angle. Local irradiation resulted in detachment of corneal epithelium, interruption of the normal trabecular structure of the ciliary process, and confluence of the lymphatic lacunae of Fontana. At a later stage, blood vessels and ciliary muscle show injury. After whole-body irradiation similar changes were seen in the ciliary body, which incurred more injury than other ocular structures. It is suggested that the primary damage to the eye resulting in increased intraocular pressure occurs here. Since the ciliary body is responsible for formation of ocular fluid and regulation of its exchange, disruption of its function by irradiation injury may result in increased pressure of this fluid.
