Correlation of radiosensitivity testing with therapeutic outcome in cervical carcinoma

BS>An investigation was made of the validity of early postradiation biopsy of carcinoma of the cervix to predict the ultimate response of individual patients to radiotherapy. This was suggested as a guide in selecting cases for early supplementary surgical excision when a poor prognosis from radiotherapy alone is anticipated. A high rate of salvage by supplementary surgical excision after clinically demonstrated failure of radiotherapy, and after recurrence, was previously reported. In 55 cases of cervical carcinoma treated with radiotherapy a study was made to detect a correlation between postradiation radiosensitivity testing (RST) and ultimate therapeutic response. Most of these cases received both Ra (average 4700 mg-hr) and x-ray treatment (average 4200 r to the pelvis). The histologic criteria used required for a diagnosis of good RST, a definite increase in differentiation of cells in the fingerlike growing edges of tumor in the postradiation biopsy. Considering all stages of cervical carcinoma together, the correlation of RST with subsequent response to radiotherapy was slightly poorer than chance alone (50%); a good RST being followed by a good therapeutic result in 48% of cases. A poor RST was followed by a poor therapeutic result in 29% of cases. As in previous studies, the stage of the cancer had a much better correlation with the therapeutic result than did radiosensitivity. Thus, under conditions of this study, RST did not prove useful in following the patient with carcinoma of the cervix treated with radiotherapy.
