Comparison of TC-99M methoxyisobutlyisonitrile (MIBI) Tl-201 and CT/MRI in the evaluation of primary tumors of the brain

Tl-201 has been used to evaluate primary brain tumors with some success reported in tumor grading and detection of recurrence. Limited information is available relative to its use in brain tumors. The purpose of the current paper was to compare MIBI with Tl-201, as well as CT/MRI in evaluating pts. with primary brain malignancies. 28 pts. with known brain tumors underwent MIBI and Tl-201 scintigraphy. 3 mCi Tl-201 planar images were begun 10 min. post-injection, and were followed by 30 mCi MIBI planar and then SPECT images. All results were compared with CT/MRI and surgery. MIBI/Planar MIBI/SPECT TL/Planar CT/MRI Sens. 65% (15/23) 71% (15/21) 50% (11/22) 95% 23/24 Spec. 100% (5/5) 80% (4/5) 100% (5/5) 50% 2/4 Tl-201 sensitivity was inferior to MIBI planar and MIBI SPECT imaging. Both MIBI and Tl-201 sensitivity was less than CT/MRI. Tumor recurrence was detected with both high sensitivity and specificity with MIBI. Lesion size and extent of tumor necrosis appeared to be important factors in detection of tumor by MIBI and Tl-201. MIBI SPECT imaging is superior to MIBI planar and Tl-201 imaging. Tl-201 sensitivity was significantly less than MIBI planar imaging (p<0.05). MIBI and Tl-201 are insensitive in detecting primary tumors of the brain when the tumors are necrotic or when the size is less than the equiv. of a 9 mm sphere.
