$sup 131$I-HSA cisternography and $sup 131$I-HSA transfer from CSF to blood in normal pressure hydrocephalus

Experience of isotope cistemography with /sup 131/I-HSA in normal pressure hydrocephalus is reported. The results lead to the following considerations: The most characteristic cisternographic pattern of normal pressure hydrocephalus (determined by ventricular filling in the absence of demonstration of CSF pathways, no activity over the convexities) is not the only pattenn existing. Some other pictures likely to be found where ventricular filling is associated with activity in the cisterns and through the cortical subarachnoid channels, sometimes reaching the convexities. All the patterns examined, though morphologically different, seemed to be related by a common dynamic feature, consisting in delayed diffusion and reabsorption of the isotope which tend to have a prolonged retention in the liquoral spaces. The examination of the plasmatic levels of the /sup 131/I-HSA supports these data; in blood samples examined from the first to the 48th hr during isotope cisternography, the authors observed, in normal pressure hydrocephalus, a plasmatic concentration of / sup 131/I-HSA 30 to 50% lower than in normal cases. The analysis of the curve of blood concentration of /sup 131/I-HSA provides useful information as far as shunting and post-operative prognosis are concerned. (auth)
