Measurement of circulatory dynamics in the liver by radioisotopes (in Japanese)

In order to examine the dynamics of hepatic circulation, radioisotope (RI) methods are described for measuring hepatic blood flow, hepatic circulation time (circulation time from the spleen to the liver, and the starting time of the circulation through the portal vein and through the hepatic artery), ratio of the blood flow through the portal vein to that through the hepatic artery, and intra- and extrahepatic shunt and shunted blood flow. The liver accumulation coefficient (K/sup L/) and disappearance coefficient in the blood (Kp) are reported to measure the hepatic blood flow. The patients, in whom the value of K/ sub L/ was significantly different from that of normal group, were those with liver cirrhosis. In healthy persons, Kp was almost equal to K/sub L/, but in advanced disturbances of the liver parenchyma cells and the portal circulation, such as liver cirrhosis, Kp was higher than K/sub L/. The dynamics of the portal circulation was revealed relatively easily and within a short time by examining the circulation time from the spleen to the liver, and information concerning the complications, particularly esophageal varices, could be obtained. No evident results were obtained by the measurement of the starting time of the circulation through the hepatic artery and the portal vein. lnjection into the hepatic vein, pulmonary arterial catheterization (/sup 131/I-HSA was used) and catheterization of the portal vein through the umbilical cord were tried to examine the ratio of hepatic arterial blood flow. The characteristics of the method of extrahepatic shunt (RIS, intrasplenic injection) and intrahepatic shunt (/sup 14/C-d galactose method) and portal perfusion scanning by heatdenatured human serum albumin labeled with /sup 131/I (/sup 132/I-macroaggregated albumin) were described to detect the intra- and extrahepatic shunt and the shunted blood flowB (JA)
