THYMIC IRRADIATION: CARCINOMA FOLLOWS INFANT THERAPY

Induction of carcinomas after thymic irradiation is reviewed, and another instance of thyroid carcinoma occurring in an adolescent patient with a history of radiotherapy during infancy is recorded. A 16-year-old girl was admitted to hospital for evaluation of a right cervical mass in 1960. This mass had been present and enlarging for at least three months, cervical adenopathy on the right side had been noted three years prior to admission. There were no mechanical or metabolic symptoms referrable to the thyroid at the time of admission. Review of her medical history revealed thymic irradiation for stridor and an enlarged thymus in the first year of life, and later irradiation of the neck for adenitis at age five (the amount of irradiation is unknown). At surgery an adenocarcinoma of the thyroid gland was revealed by frozen section, and lymph nodes were found to contain metastatic carcinoma. Histologic appearance of the tumor is described. The final histologic diagnosis was papillary adenocarcinoma of the right lobe of the thyroid, with metastasis to six lymph nodes of the neck and anterior mediastinum. The patient was followed for 36 months and apparently remains free of recurrent cancer. (BBB)
