ANALYSIS OF A SERIES OF CASES OF CARCINOMA OF THE ENDOMETRIUM TREATED BY RADIUM AND OPERATION: A 40-YEAR EXPERIENCE AT WOMAN'S HOSPITAL

A survey of 810 cases of endometrial cancer treated since 1919 when the first Ra was obtained, is presented. There were 50 cases of the 610 which received Ra treatment for benign disease from 1 to 20 yr prior to the development of cancer, an incidence of 8.2%. The indications for Ra treatment were menorrhagia, metrorrhagia, hyperplasia, fibromyomas, and polyps. The dosage varied from 1200 mg-hr to 3600 mg-hr. Two patients with carcinoma of the cervix who received 4200 mg-hr of Ra subsequently developed carcinoma of the endometrium (1200 cases of cervical cancer were treated with Ra since 1919). Thus 0.01% developed cancer of the endometrium after large doses of radium, whereas 2.3% of the total number that received small doses of Ra for benign disease developed cancer of the endometrium. This indicates that large doses of Ra do not necessarily produce endometrial cancer but that small doses may do so. The majority of the 413 endometrial cancer cases receiving Ra were treated with 1800 to 2400 mg-hr doses; the most common dosage was 2400 mg-hr, which is very low according to present-day standards. The survival of the patients was more clearly related to the histological type and stage of cancer than the treatment per se. The 5-hr survival rate according to treatment showed a 31.9% survival in those treated with Ra and/or x rays. Vaginal hysterectomies gave only a 25% 5-yr survival, but subtotal hysterectomies showed a 53.6% survival rate. The primary hysterectomy overall survival rate was 65.3% and cases treated by preoperative Ra followed by hysterectomy, 88.3%. The difference in these last 2 groups is not statistically significant.
