Root cause analysis in EHS auditing in oil & gas production

Mobil Exploration & Producing United States Inc. (MEPUS) utilizes internal auditing for assisting in Environmental, Health, & Safety (EHS) compliance. This paper addresses the Midland North Asset Team`s (MNAT) Level III EHS Audit Program process and methodology of analyzing audit results. Audits are performed upon every well, lease, and facility usually every three years, with findings formally identified, and corrective actions planned and initiated. Audit results are analyzed using a system that prioritizes findings, and divides results into Loss Prevention and Environmental issues. Pareto charts are formed utilizing the subdivisions and prioritizations. Key Performance Indicators (KPIs) used include, Total Number of Findings, Total Number of Priority {open_quotes}A{close_quotes} Findings, Percentage of Findings Corrected On-Time, Number of Months to Correct All Findings, and {open_quotes}AFACTOR{close_quotes}s. Root cause analysis is also performed, employing a unique Staged Pareto investigation that not only demonstrates source reasons for audit findings, but also tracks the shifts from a {open_quotes}beginning{close_quotes} EHS program to a more {open_quotes}intermediate and advancing{close_quotes} program.
