Cortical activation studies with technetium 99m ECD: Same-day vs. different-day protocols

Using technetium 99m ethyl cysteinate dimer (ECD) to study regional cerebral blood flow (rCBF) changes during cortical activation, this study compares a different-day with a same-day protocol. Normal volunteers (n=10) had two SPECT (SME 810) studies with ECD (20 millicuries), each on separate days. Another group of normal volunteers (n=10) had the two studies performed on the same day. In each subject, a baseline study (dark room with eyes open), was compared to a full field visual activation (flashing checkerboard) study. The paired studies were realigned and coregistered with an automated program. Regional analysis of the visual cortex was compared to the whole brain activity. The different-day studies, were compared to the same-day studies. In all subjects, visual stimulation caused easily observed visual cortex activation, and the mean data demonstrated the rCBF change to be statiscally significant (different-day: 7.22 {plus_minus} 2.2 vs same-day: 5.85 {plus_minus}3.5). The same day studies were re-analyzed with decay correction for the baseline activity (11.22 {plus_minus}6.6). Group mean data and individual patient studies for all subjects showed quantitative changes in the visual cortex. SPECT visual activation studies using ECD can be performed with different-day or same-day protocols. Further characterization of background activity may enhance the changes on same-day protocols. These methods can be applied to other cortical activation paradigms.
