Title: Fixing the right problem: System advances in DOE procedures

When the US Department of Energy (DOE) Headquarters Office of Nuclear Safety set out in 1990 to review procedure use throughout the DOE nuclear complex, they found that by and large, procedures had a bad press. Too many DOE personnel and DOE contractors had encountered procedures that either failed to make their work safer and easier or actually made it more difficult. This is ironic as well as frustrating because procedures are job aids, i.e., information sources people are supposed to be able to rely on to do their jobs better. Few people encountered in the complex felt they had access to procedures that consistently helped them do their jobs better.
