THE PURPOSE OF THIS PAPER IS TO ANALYZE THE RESULTS IN 723 PATIENTS WITH DUODENAL ULCER TREATED WITH IRRADIATION AND OBSERVED FOR 5 TO 18 YEARS. WITH THE PROGRAM OF MEDICAL TREATMENT DESCRIBED, INCLUDING EXTERNAL IRRADIATION DIRECTED TO THE FUNDUS AND BODY OF THE STOMACH, THE RECURRENCE RATE IN TERMS OF EPISODES PER 100 YEARS DECREASED FROM 102 TO 17; THE RECURRENCE RATE FOR HEMORRHAGE DIMINISHED FORM 4.3 TO 1.1; AND THE INCIDENCE OF PERFORATION DECREASED FROM 0.2 TO 0.1. EIGHT DEATHS WERE ATTRIBUTABLE TO ULCERS. ONE HUNDRED FIFTEEN REQUIRED SURGERY WITHIN THE FOLLOW-UP PERIOD OF 5 TO 18 YEARS. OF THE 723 PATIENTS, 114 ARE KNOWN TO BE DEAD. THE CAUSES OF DEATH ARE ANALYZED WITH PARTICULAR REFERENCE TO CARCINOMA AND THE EFFECTS OF RADIATION. THE SIGNIFICANCE OF THE 6 KNOWN DEATHS FROM CARCINOMA OF THE STOMACH, PANCREAS AND DUODENUM DEVELOPING 2 TO 13 YEARS AFTER TREATMENT IS YET TO BE DETERMINED. ROENTGEN IRRADIATION IN MODERATE AMOUNTS TO THE ACID-SECRETING AREAS OF THE STOMACH CONSTITUTES A SAFE AND VALUABLE ADJUNCT IN THE TREATMENT OF DUODENAL ULCER.
