Description/Abstract:
IN THE YEARS 1918, 1919, AND 1920, 175 CASES OF CARCINOMA OF THE BREAST ENTERED THE WARDS OF THE MASSACHUSETTS GENERAL HOSPITAL IN WHICH SURGERY WAS PERFORMED. DURING THIS PERIOD, ROENTGEN RADIATION WAS BEGINNING TO BE USED AS A ROUTINE BEFORE-AND-AFTER OPERATION, AS A PROPHYLACTIC MEASURE. ABOUT ONE-HALF THE CASES OF RADICAL OPERATION (REMOVAL OF THE WHOLE BREAST, THE SKIN OVER IT, BOTH PECTORAL MUSCLES, THE AXILLARY CONTENTS AND THE DEEP FASCIA) IN THIS SERIES HAD ROENTGEN TREATMENT EITHER BEFORE OR AFTER OPERATION, OR BOTH. THE RESULTS AFTER 5 YEARS ARE DISCUSSED. OF THE 134 PRIMARY CASES, 21 ARE KNOWN TO BE ALIVE AND WELL, OR 16 PERCENT. IN 10 CASES NO OPERATION AT WAS DONE; IN 24 CASES ONLY AN INCOMPLETE OPERATION WAS PERFORMED AS A PALLIATIVE MEASURE, AND IN 6 CASES RADICAL OPERATION WAS ATTEMPTED IN PATIENTS TOO FAR ADVANCED FOR CURE. FOUR PATIENTS DIED SHORTLY AFTER OPERATION. FOUR OTHER PATIENTS DIED OF OTHER DISEASES BETWEEN 3 AND 5 YEARS AFTER OPERATION, WITHOUT RECURRENCE; AND 23 CASES CANNOT BE TRACED TO THE FIVE-YEAR LIMIT. IN THE 76 RADICAL OPERATIONS 5-YEAR RESULTS, 70 SHOWED NO EVIDENCE OF DISEASE BEYOND THE LIMITS OF OPERATIVE REMOVAL WHILE 6 WERE ADVANCED CASES. TWENTY-ONE CASES ARE KNOWN TO BE ALIVE AND WELL FIVE OR MORE YEARS AFTER THE OPERATION. THE PERCENTAGE OF CURE WITH AND WITHOUT ROENTGEN IRRADIATION IS EXACTLY THE SAME.
