GRC accomplishments are: (1) Failure database software deployed - partners see business value for themselves and customers; (2) Designed, built, instrumented, and tested two gearboxes - (a) Generated unprecedented public domain test data from both field testing and dynamometer testing, (b) Different responses from 'identical' gearboxes, (c) Demonstrated importance of non-torque loading and modeling approach; and (3) Active collaborative, with wide industry support, leveraging DOE funding - Modeling round robin and Condition Monitoring round robin.

    Geothermal resources with temperatures between 250/sup 0/ and 360/sup 0/F (121/sup 0/C and 182/sup 0/C) are prime candidates for binary-cycle power generation, and constitute about 80% of the power-capable resources in the United States. The successful exploitation of these resources requires reliable high-capacity downwell brine production pumps, but earlier experience showed that high-capacity, high-temperature geothermal production pumps had many problems which resulted in a mean time-to-failure (MTTF) of less than 1000 h. However, steady progress has been made since 1981, and a large body of experience has been acquired by three geothermal binary plants. This survey of high-temperature geothermal downwell pump users and manufacturers updates a prior survey (AP-3572) completed in early 1983. This survey traces the development of lineshaft pump technology from the late 1970s to the present (mid-1987), detailing the advances in design, materials selection, and operating practices. Case histories of 72 lineshaft pumps installed at three geothermal binary plants since late 1981 are documented, including some detailed cause of failure reports. In the recent past, pump lives in excess of 7000 h have become common, but a high continuing rate of premature failures resulted in a mean time-to-failure (MTTF) of about 5000 h. Based on recent advances which appear likely to eliminate most premature failures, the estimated near-term MTTF will be on the order of 8000 h. The survey found almost no development of high-temperature geothermal electric submersible pumps (ESP's) or close-coupled downwell hydraulic turbopumps, and concluded that considerable development and demonstration will be needed before these technologies are able to compete with existing high-temperature geothermal lineshaft pump technology. 36 refs., 10 figs., 25 tabs.

    Process Safety Management (PSM) regulation applies mainly to chemical and oil companies, plus other industries such as food processing, and paper production. It covers plants that deal with any of about 130 toxic chemicals in quantities above thresholds limits in Appendix A of PSM standard or flammable liquids and gases in quantities greater than 10,000 pounds. The State Consultation PSM teams (North and South) use a detailed audit checklist featuring above elements for audit of facilities requesting PSM assistance. The audit focuses on three generic activities: records, onsite conditions, and interviews. Records include written programs that outline the requirements and documentation of completion. Onsite conditions are implementation of activities specified in written programs. Interviews are for evaluation of transferred information to the workers that are already developed in written programs and training programs.

  4. HFE Process Guidance and Standards for potential application to updating NRC guidance

    The U.S. Nuclear Regulatory Commission (NRC) reviews and evaluates the human factors engineering (HFE) programs of applicants for nuclear power plant construction permits, operating licenses, standard design certifications, and combined operating licenses. The purpose of these safety reviews is to help ensure that personnel performance and reliability are appropriately supported. Detailed design review procedures and guidance for the evaluations is provided in three key documents: the Standard Review Plan (NUREG-0800), the HFE Program Review Model (NUREG-0711), and the Human-System Interface Design Review Guidelines (NUREG-0700). These documents were last revised in 2007, 2004 and 2002, respectively. The NRC is committed to the periodic update and improvement of these guidance documents to ensure that they remain state-of-the-art design evaluation tools. Thus, the NRC has initiated a project with BNL to update the NRC guidance to remain current with recent research on human performance, advances in HFE methods and tools, and new technology. INL supported Brookhaven National Lab (BNL) to update the detailed HFE review criteria contained in NUREG-0711 and NUREG-0700 based on (1) feedback obtained from end users, (2) the results of NRC research and development efforts supporting the NRC staff’s HFE safety reviews, and (3) other material the project staff identify as applicable to the update effort. INL submitted comments on development plans and sections of NUREGs 0800, 0711, and 0700. The contractor prepared the report attached here as the deliverable for this work.

    An International Atomic Energy Agency (IAEA) Data Development Project was initiated to provide a mechanism for allowing new experimental data and improvements in evaluation procedures to be incorporated into new evaluations of the standards. The work on this project is ongoing. In the past very long periods sometimes occurred between evaluations of the standards. Through the use of this Project, such long periods should not occur. Work on the cross section standards through this Project has included an update of the experimental data to be used in the cross section standards evaluations, a study of the uncertainties obtained in the international standards evaluation, and improvements in the smoothing procedure for capture cross sections. It was decided that this Project should have a broader range of activities than just the cross section standards and thus encompass standards related activities. The following are being investigated: improvements in the gold cross section at energies below where it is considered a standard and work on certain cross sections that are not as well known as the cross section standards but could be very useful as reference cross sections relative to which certain types of cross section measurements can be made. This work includes prompt gamma-ray production in fast neutron-induced reactions, and work on the {sup 252}Cf spontaneous fission neutron spectrum and the {sup 235}U thermal neutron fission spectrum. Most of the data investigated through this Project are used in dosimetry applications. (authors)

    The Standard Review Plan (SRP) (Reference 2), provides guidance to the regulatory staff of the Nuclear Regulatory Commission (NRC) on performing their safety reviews of applications to construct or operate nuclear power plants, and applications to approve standard designs and sites for nuclear power plants. Chapter 8 of the SRP provides guidance related to the review of station electrical distribution systems described by the applicant in its Design Control Document (DCD) or Safety Analysis Report (SAR). As part of the 2006-2007 SAR update, all sections in this Chapter (8.1, 8.2, 8.3.1, 8.3.2, Appendix 8A, and Appendix 8B) were revised to incorporate new analyses, design approaches, and the lessons learned from the review of the AP 1000 design certification and to assure consistency with the draft Regulatory Guide DG-1145, ''Combined License Applications for Nuclear Power Plants (LWR Edition)''.

These software accuracy test cases and their example simulation results, which comprise the first test suite adapted for the initial 2001 version of Standard 140, are approaching their 20th anniversary. In response to the evolution of the state of the art in building thermal fabric modeling since the test cases and example simulation results were developed, work is commencing to update the normative test specification and the informative example results.

    The driving force underlying creation of mandatory reliability standards was the prevention of widespread outages, such as those that occurred in 1965, 1977 and 2003. So far, no similar outage has occurred when an entity is in full compliance with the standards, and NERC and FERC have demonstrated that they will actively enforce compliance while aggressively pursuing entities alleged to be non-compliant.     The state of California was given authority under the Clean Air Act Amendments of 1990 (CAAA90) to set emissions standards for light-duty vehicles that exceed federal standards. In addition, other states that do not comply with the National Ambient Air Quality Standards (NAAQS) set by the Environmental Protection Agency under CAAA90 were given the option to adopt Californias light-duty vehicle emissions standards in order to achieve air quality compliance. CAAA90 specifically identifies hydrocarbon, carbon monoxide, and NOx as vehicle-related air pollutants that can be regulated. California has led the nation in developing stricter vehicle emissions standards, and other states have adopted the California standards.

    The impact of recent changes to the ASTM Standard E722 is investigated. The methodological changes in the production of the displacement kerma factors for silicon has significant impact for some energy regions of the 1-MeV(Si) equivalent fluence response function. When evaluating the integral over all neutrons energies in various spectra important to the SNL electronics testing community, the change in the response results in an increase in the total 1-MeV(Si) equivalent fluence of 2 7%. Response functions have been produced and are available for users of both the NuGET and MCNP codes.

    This report addresses Hawaii electric system reliability issues; greater emphasis is placed on short-term reliability but resource adequacy is reviewed in reference to electric consumers' views of reliability %E2%80%9Cworth%E2%80%9D and the reserve capacity required to deliver that value. The report begins with a description of the Hawaii electric system to the extent permitted by publicly available data. Electrical engineering literature in the area of electric reliability is researched and briefly reviewed. North American Electric Reliability Corporation standards and measures for generation and transmission are reviewed and identified as to their appropriateness for various portions of the electric grid and for application in Hawaii. Analysis of frequency data supplied by the State of Hawaii Public Utilities Commission is presented together with comparison and contrast of performance of each of the systems for two years, 2010 and 2011. Literature tracing the development of reliability economics is reviewed and referenced. A method is explained for integrating system cost with outage cost to determine the optimal resource adequacy given customers' views of the value contributed by reliable electric supply. The report concludes with findings and recommendations for reliability in the State of Hawaii.

    A laser safety and hazard analysis was performed for the temperature stabilized Big Sky Laser Technology (BSLT) laser central to the ARES system based on the 2007 version of the American National Standards Institute's (ANSI) Standard Z136.1, for Safe Use of Lasers and the 2005 version of the ANSI Standard Z136.6, for Safe Use of Lasers Outdoors. The ARES laser system is a Van/Truck based mobile platform, which is used to perform laser interaction experiments and tests at various national test sites.

    Solar photovoltaics (PV) is the dominant type of distributed generation (DG) technology interconnected to electric distribution systems in the United States, and deployment of PV systems continues to increase rapidly. Considering the rapid growth and widespread deployment of PV systems in United States electric distribution grids, it is important that interconnection procedures be as streamlined as possible to avoid unnecessary interconnection studies, costs, and delays. Because many PV interconnection applications involve high penetration scenarios, the process needs to allow for a sufficiently rigorous technical evaluation to identify and address possible system impacts. Existing interconnection procedures are designed to balance the need for efficiency and technical rigor for all DG. However, there is an implicit expectation that those procedures will be updated over time in order to remain relevant with respect to evolving standards, technology, and practical experience. Modifications to interconnection screens and procedures must focus on maintaining or improving safety and reliability, as well as accurately allocating costs and improving expediency of the interconnection process. This paper evaluates the origins and usefulness of the capacity penetration screen, offers potential short-term solutions which could effectively allow fast-track interconnection to many PV system applications, and considers longer-term solutions for increasing PV deployment levels in a safe and reliable manner while reducing or eliminating the emphasis on the penetration screen.

    The U.S. Nuclear Regulatory Commission (NRC) reviews the human factors engineering (HFE) programs of applicants for nuclear power plant construction permits, operating licenses, standard design certifications, and combined operating licenses. The purpose of these safety reviews is to help ensure that personnel performance and reliability are appropriately supported. Detailed design review procedures and guidance for the evaluations is provided in three key documents: the Standard Review Plan (NUREG-0800), the HFE Program Review Model (NUREG-0711), and the Human-System Interface Design Review Guidelines (NUREG-0700). These documents were last revised in 2007, 2004 and 2002, respectively. The NRC is committed to the periodic update and improvement of the guidance to ensure that it remains a state-of-the-art design evaluation tool. To this end, the NRC is updating its guidance to stay current with recent research on human performance, advances in HFE methods and tools, and new technology being employed in plant and control room design. This paper describes the role of HFE guidelines in the safety review process and the content of the key HFE guidelines used. Then we will present the methodology used to develop HFE guidance and update these documents, and describe the current status of the update program.

    This workshop was the sixth in a series of workshops sponsored by NREL/DOE under the general subject of photovoltaic testing and reliability during the period 1986--1993. PV performance and PV reliability are at least as important as PV cost, if not more. In the US, PV manufacturers, DOE laboratories, electric utilities, and others are engaged in the photovoltaic reliability research and testing. This group of researchers and others interested in the field were brought together to exchange the technical knowledge and field experience as related to current information in this evolving field of PV reliability. The papers presented here reflect this effort since the last workshop held in September, 1992. The topics covered include: cell and module characterization, module and system testing, durability and reliability, system field experience, and standards and codes.

    This proceedings is the compilation of papers presented at the ninth PV Performance and Reliability Workshop held at the Sheraton Denver West Hotel on September 4--6, 1996. This years workshop included presentations from 25 speakers and had over 100 attendees. All of the presentations that were given are included in this proceedings. Topics of the papers included: defining service lifetime and developing models for PV module lifetime; examining and determining failure and degradation mechanisms in PV modules; combining IEEE/IEC/UL testing procedures; AC module performance and reliability testing; inverter reliability/qualification testing; standardization of utility interconnect requirements for PV systems; need activities to separate variables by testing individual components of PV systems (e.g. cells, modules, batteries, inverters,charge controllers) for individual reliability and then test them in actual system configurations; more results reported from field experience on modules, inverters, batteries, and charge controllers from field deployed PV systems; and system certification and standardized testing for stand-alone and grid-tied systems.

    This report provides the DOE and industry with a general process for analyzing power electronics reliability. The analysis can help with understanding the main causes of failures, downtime, and cost and how to reduce them. One approach is to collect field maintenance data and use it directly to calculate reliability metrics related to each cause. Another approach is to model the functional structure of the equipment using a fault tree to derive system reliability from component reliability. Analysis of a fictitious device demonstrates the latter process. Optimization can use the resulting baseline model to decide how to improve reliability and/or lower costs. It is recommended that both electric utilities and equipment manufacturers make provisions to collect and share data in order to lay the groundwork for improving reliability into the future. Reliability analysis helps guide reliability improvements in hardware and software technology including condition monitoring and prognostics and health management.

    This report provides a distribution reliability measurement ''toolkit'' that is intended to be an asset to regulators, utilities and power users. The metrics and standards discussed range from simple reliability, to power quality, to the new blend of reliability and power quality analysis that is now developing. This report was sponsored by the Office of Electric Transmission and Distribution, U.S. Department of Energy (DOE). Inconsistencies presently exist in commonly agreed-upon practices for measuring the reliability of the distribution systems. However, efforts are being made by a number of organizations to develop solutions. In addition, there is growing interest in methods or standards for measuring power quality, and in defining power quality levels that are acceptable to various industries or user groups. The problems and solutions vary widely among geographic areas and among large investor-owned utilities, rural cooperatives, and municipal utilities; but there is still a great degree of commonality. Industry organizations such as the National Rural Electric Cooperative Association (NRECA), the Electric Power Research Institute (EPRI), the American Public Power Association (APPA), and the Institute of Electrical and Electronics Engineers (IEEE) have made tremendous strides in preparing self-assessment templates, optimization guides, diagnostic techniques, and better definitions of reliability and power quality measures. In addition, public utility commissions have developed codes and methods for assessing performance that consider local needs. There is considerable overlap among these various organizations, and we see real opportunity and value in sharing these methods, guides, and standards in this report. This report provides a ''toolkit'' containing synopses of noteworthy reliability measurement practices.     The paper and presentations compiled in this volume form the Proceedings of the fourth in a series of Workshops sponsored by Solar Energy Research Institute (SERI/DOE) under the general theme of photovoltaic module reliability during the period 1986--1990. The reliability Photo Voltaic (PV) modules/systems is exceedingly important along with the initial cost and efficiency of modules if the PV technology has to make a major impact in the power generation market, and for it to compete with the conventional electricity producing technologies. The reliability of photovoltaic modules has progressed significantly in the last few years as evidenced by warranties available on commercial modules of as long as 12 years. However, there is still need for substantial research and testing required to improve module field reliability to levels of 30 years or more. Several small groups of researchers are involved in this research, development, and monitoring activity around the world. In the US, PV manufacturers, DOE laboratories, electric utilities and others are engaged in the photovoltaic reliability research and testing. This group of researchers and others interested in this field were brought together under SERI/DOE sponsorship to exchange the technical knowledge and field experience as related to current information in this important field. The papers presented here reflect this effort.

    The reexamination of astrophysical opacities has eliminated gross discrepancies between a variety of observations and theoretical calculations; thus allowing for more detailed tests of stellar models. A number of such studies indicate that model results are sensitive to modest changes in the opacity. Consequently, it is desirable to update available opacity databases with recent improvements in physics, refinements of element abundance, and other such factors affecting the results. Updated OPAL Rosseland mean opacities are presented. The new results have incorporated improvements in the physics and numerical procedures as well as corrections. The main opacity changes are increases of as much as 20{percent} for Population I stars due to the explicit inclusion of 19 metals (compared to 12 metals in the earlier calculations) with the other modifications introducing opacity changes smaller than 10{percent}. In addition, the temperature and density range covered by the updated opacity tables has been extended. As before, the tables allow accurate interpolation in density and temperature as well as hydrogen, helium, carbon, oxygen, and metal mass fractions. Although a specific metal composition is emphasized, opacity tables for different metal distributions can be made readily available.     A Human Reliability Program (HRP) is designed to protect national security as well as worker and public safety by continuously evaluating the reliability of those who have access to sensitive materials, facilities, and programs. Some elements of a site HRP include systematic (1) supervisory reviews, (2) medical and psychological assessments, (3) management evaluations, (4) personnel security reviews, and (4) training of HRP staff and critical positions. Over the years of implementing an HRP, the Department of Energy (DOE) has faced various challenges and overcome obstacles. During this 4-day activity, participants will examine programs that mitigate threats to nuclear security and the insider threat to include HRP, Nuclear Security Culture (NSC) Enhancement, and Employee Assistance Programs. The focus will be to develop an understanding of the need for a systematic HRP and to discuss challenges and best practices associated with mitigating the insider threat.

    This report provides an update on the valve research sponsored by the US Nuclear Regulatory Commission (NRC) that is being conducted at the Idaho National Engineering Laboratory. The update focuses on the information applicable to the following requests from the NRC staff: Examine the use of in situ tests results to estimate the response of a valve at design-basis conditions; Examine the methods used by industry to predict required valve stem forces or torques; Identify guidelines for satisfactory performance of motor-operated valve diagnostics systems; and participate in writing a performance standard or guidance document for acceptable design-basis tests. The authors have reviewed past, current, and ongoing research programs to provide the information available to address these items.

    During FY 1989, the DOE-Oak Ridge Operations (ORO) office formed joint Operating Contractor/DOE-ORO organizations to address safety analysis related issues. The Safety Analysis Report Working Group (SARWG) took on the task of developing a strategy to address the issue of updating SARs to today's standards. The resulting SAR Update Program was approved by the Safety Analysis Report Management Group (SARMG) and on November 6, 1989, was accepted by the senior management of DOE-ORO, and its operating contractors, including Martin Marietta Energy Systems, Inc. This SAR Update Program consists of five phases: Phase 0 -- continued operation evaluations; Phase 1 -- hazard classification and qualitative analysis; Phase 1A -- updated operational safety requirements; Phase 2 -- quantitative accident analysis; and, Phase 3 -- complete DOE-approved SARs. 8 refs., 17 figs., 6 tabs.

    The Blade Reliability Collaborative (BRC) was started by the Wind Energy Technologies Department of Sandia National Laboratories and DOE in 2010 with the goal of gaining insight into planned and unplanned O&M issues associated with wind turbine blades. A significant part of BRC is the Blade Defect, Damage and Repair Survey task, which will gather data from blade manufacturers, service companies, operators and prior studies to determine details about the largest sources of blade unreliability. This report summarizes the initial findings from this work.

    This report summarizes the results of the Load Control System Reliability project (DOE Award DE-FC26-06NT42750). The original grant was awarded to Montana Tech April 2006. Follow-on DOE awards and expansions to the project scope occurred August 2007, January 2009, April 2011, and April 2013. In addition to the DOE monies, the project also consisted of matching funds from the states of Montana and Wyoming. Project participants included Montana Tech; the University of Wyoming; Montana State University; NorthWestern Energy, Inc., and MSE. Research focused on two areas: real-time power-system load control methodologies; and, power-system measurement-based stability-assessment operation and control tools. The majority of effort was focused on area 2. Results from the research includes: development of fundamental power-system dynamic concepts, control schemes, and signal-processing algorithms; many papers (including two prize papers) in leading journals and conferences and leadership of IEEE activities; one patent; participation in major actual-system testing in the western North American power system; prototype power-system operation and control software installed and tested at three major North American control centers; and, the incubation of a new commercial-grade operation and control software tool. Work under this grant certainly supported the DOE-OE goals in the area of “Real Time Grid Reliability Management.”

    Observations of metal-poor extragalactic H II regions allow the determination of the primordial helium abundance, Y{sub p}. The He I emissivities are the foundation of the model of the H II region's emission. Porter, Ferland, Storey, and Detisch (2012) have recently published updated He I emissivities based on improved photoionization cross-sections. We incorporate these new atomic data and update our recent Markov Chain Monte Carlo analysis of the dataset published by Izotov, Thuan, and Stasi'nska (2007). As before, cuts are made to promote quality and reliability, and only solutions which fit the data within 95% confidence level are used to determine the primordial He abundance. The previously qualifying dataset is almost entirely retained and with strong concordance between the physical parameters. Overall, an upward bias from the new emissivities leads to a decrease in Y{sub p}. In addition, we find a general trend to larger uncertainties in individual objects (due to changes in the emissivities) and an increased variance (due to additional objects included). From a regression to zero metallicity, we determine Y{sub p} = 0.2465 ± 0.0097, in good agreement with the BBN result, Y{sub p} = 0.2485 ± 0.0002, based on the Planck determination of the baryon density. In the future, a better understanding of why a large fraction of spectra are not well fit by the model will be crucial to achieving an increase in the precision of the primordial helium abundance determination.

    Wind industry has been challenged by premature subsystem/component failures. Various reliability data collection efforts have demonstrated their values in supporting wind turbine reliability and availability research & development and industrial activities. However, most information on these data collection efforts are scattered and not in a centralized place. With the objective of getting updated reliability statistics of wind turbines and/or subsystems so as to benefit future wind reliability and availability activities, this report is put together based on a survey of various reliability databases that are accessible directly or indirectly by NREL. For each database, whenever feasible, a brief description summarizing database population, life span, and data collected is given along with its features & status. Then selective results deemed beneficial to the industry and generated based on the database are highlighted. This report concludes with several observations obtained throughout the survey and several reliability data collection opportunities in the future.

    One emerging application for ceramics is diesel particulate filters being used order to meet EPA regulations going into effect in 2008. Diesel particulates are known to be carcinogenic and thus need to be minimized. Current systems use filters made from ceramics such as mullite and corderite. The filters are brittle and must operate at very high temperatures during a burn out cycle used to remove the soot buildup. Thus the filters are subjected to thermal shock stresses and life time reliability analysis is required. NASA GRC has developed reliability based design methods and test methods for such applications, such as CARES/Life and American Society for Testing and Materials (ASTM) C1499 “Standard Test Method for Equibiaxial Strength of Ceramics.”

    This document highlights the important results obtained from the subtask of the Goodyear CRADA devoted to better understanding reliability of tires and to developing better lifetime prediction methods. The overall objective was to establish the chemical and physical basis for the degradation of tires using standard as well as unique models and experimental techniques. Of particular interest was the potential application of our unique modulus profiling apparatus for assessing tire properties and for following tire degradation. During the course of this complex investigation, extensive relevant information was generated, including experimental results, data analyses and development of models and instruments. Detailed descriptions of the findings are included in this report.

    Terminological requirements in information management was but one of the principal themes of the 2nd Congress on Terminology and Knowledge Engineering. The traveler represented the American Society for Testing and Materials' Committee on Terminology, of which he is the Chair. The traveler's invited workshop emphasized terminology standardization requirements in databases of material properties as well as practical terminology standardizing methods. The congress included six workshops in addition to approximately 82 lectures and papers from terminologists, artificial intelligence practitioners, and subject specialists from 18 countries. There were approximately 292 registrants from 33 countries who participated in the congress. The congress topics were broad. Examples were the increasing use of International Standards Organization (ISO) Standards in legislated systems such as the USSR Automated Data Bank of Standardized Terminology, the enhanced Physics Training Program based on terminology standardization in Physics in the Chinese province of Inner Mongolia, and the technical concept dictionary being developed at the Japan Electronic Dictionary Research Institute, which is considered to be the key to advanced artificial intelligence applications. The more usual roles of terminology work in the areas of machine translation. indexing protocols, knowledge theory, and data transfer in several subject specialties were also addressed, along with numerous special language terminology areas.

    NREL has initiated the Gearbox Reliability Collaborative (GRC) to investigate the root cause of the low wind turbine gearbox reliability. The GRC follows a multi-pronged approach based on a collaborative of manufacturers, owners, researchers and consultants. The project combines analysis, field testing, dynamometer testing, condition monitoring, and the development and population of a gearbox failure database. At the core of the project are two 750kW gearboxes that have been redesigned and rebuilt so that they are representative of the multi-megawatt gearbox topology currently used in the industry. These gearboxes are heavily instrumented and are tested in the field and on the dynamometer. This report discusses the bearing calibrations of the gearboxes.

    A thermal battery is a one shot device. Its overall reliability is given as the product of its technical and operational reliability. This work evaluates operational reliability. The operational reliability for various performance requirements was estimated by analyzing data received from qualification tests of a certain thermal battery. A lower bound of its operational reliability was evaluated by use of the statistical tolerance method for each specific electrical performance requirement. A conservative overall lower bound for the operational reliability of the thermal battery was calculated as the product of the individual operational reliability estimates corresponding to each performance requirement.

But changes in the focus and structure of the electric industry have put strains on that regime - as starkly revealed in the 2003 blackout. Efforts to adopt enforceable reliability standards depend on the threat of penalties to outweigh the profit incentives that undermine reliability. It is not clear that those penalties will work.

    This report provides the results of comparisons of the cited and latest versions of ANS, ASME, AWS and NFPA standards cited in the NRC Standard Review Plan for the Review of Safety Analysis Reports for Nuclear Power Plants (NUREG 0800) and related documents. The comparisons were performed by Battelle Pacific Northwest Laboratories in support of the NRC`s Standard Review Plan Update and Development Program. Significant changes to the standards, from the cited version to the latest version, are described and discussed in a tabular format for each standard. Recommendations for updating each citation in the Standard Review Plan are presented. Technical considerations and suggested changes are included for related regulatory documents (i.e., Regulatory Guides and the Code of Federal Regulations) citing the standard. The results and recommendations presented in this document have not been subjected to NRC staff review.

    Computerized procedures (CPs) are an emerging technology within nuclear power plant control rooms. While CPs have been implemented internationally in advanced control rooms, to date no US nuclear power plant has implemented CPs in its main control room (Fink et al., 2009). Yet, CPs are a reality of new plant builds and are an area of considerable interest to existing plants, which see advantages in terms of enhanced ease of use and easier records management by omitting the need for updating hardcopy procedures. The overall intent of this paper is to provide a characterization of human reliability analysis (HRA) issues for computerized procedures. It is beyond the scope of this document to propose a new HRA approach or to recommend specific methods or refinements to those methods. Rather, this paper serves as a review of current HRA as it may be used for the analysis and review of computerized procedures.

    The motivation for an increased scope and a more proactive effort in reliability research of photovoltaic modules and systems includes reducing the levelized cost of energy and gaining better confidence in the energy and financial payback for photovoltaic systems. This increased reliability and confidence will lead to greater penetration of photovoltaics in the energy portfolio and greater employment in photovoltaics and related industries. Present research needs include the fundamental degradation mechanisms of polymers, connectors and other module components, mapping of failure mechanisms observed in the field to those in accelerated lifetime tests, determining the acceleration factors, and improving standards for modules such that tests can appropriately be assigned to evaluate their long term durability. Specific mechanisms discussed are corrosion in module components, metastability in thin-film active layers, delamination and loss of elastic properties in module polymeric materials, and inverter failure. Presently, there is hiring of reliability scientists and engineers at many levels of the value chain for photovoltaics.

In addition, it will provide a special focus on the efforts to update the surface guidelines table in DOE directives. In planning to update its property clearance criteria DOE is considering many factors; such as their protectiveness, whether they are dose-based, consistency with voluntary consensus standards and national and international radiation protection recommendations, and consistency with DOE dose constraints. One option being considered is a hybrid approach using a national consensus standard for personal property and DOE derived values for real property.

    The author presents results on studies made of the reliability of steam generator (SG) tubing. The basis for this work is that in Japan the issue of defects in SG tubing is addressed by the approach that any detected defect should be repaired, either by plugging the tube or sleeving it. However, this leaves open the issue that there is a detection limit in practice, and what is the effect of nondetectable cracks on the performance of tubing. These studies were commissioned to look at the safety issues involved in degraded SG tubing. The program has looked at a number of different issues. First was an assessment of the penetration and opening behavior of tube flaws due to internal pressure in the tubing. They have studied: penetration behavior of the tube flaws; primary water leakage from through-wall flaws; opening behavior of through-wall flaws. In addition they have looked at the question of the reliability of tubing with flaws during normal plant operation. Also there have been studies done on the consequences of tube rupture accidents on the integrity of neighboring tubes.

The SRP-UDP has been completed and the SRP-Maintenance Program (SRP-MP) is now maintaining this listing. Besides updating previous information, Revision 3 adds approximately 80 citations. This listing identifies the version of the code or standard cited in the regulatory document, the regulatory document, and the current version of the code or standard. It also provides a summary characterization of the nature of the citation. This listing was developed from electronic searches of the Code of Federal Regulations and the NRC`s Bulletins, Information Notices, Circulars, Enforcement Manual, Generic Letters, Inspection Manual, Policy Statements, Regulatory Guides, Standard Technical Specifications and the Standard Review Plan (NUREG-0800).

    The reliability of some electronics components for the Deep Underwater Muon and Neutrino Detection (DUMAND) System is discussed. An introductory overview of engineering concepts and technique for reliability assessment is given. Component reliability is discussed in the contest of major factors causing failures, particularly with respect to physical and chemical causes, process technology and testing, and screening procedures. Failure rates are presented for discrete devices and for integrated circuits as well as for basic electronics components. Furthermore, the military reliability specifications and standards for semiconductor devices are reviewed.

    This document updates the 1978 report on the status of thermal imaging technology as applied to energy conservation in buildings. Thermal imaging technology is discussed in terms of airborne surveys, ground survey programs, and application needs such as standards development and lower cost equipment. Information on the various thermal imaging devices was obtained from manufacturer's standard product literature. Listings are provided of infrared projects of the DOE building diagnostics program, of aerial thermographic firms, and of aerial survey programs. (LCL)
