    A Human Reliability Program (HRP) is designed to protect national security as well as worker and public safety by continuously evaluating the reliability of those who have access to sensitive materials, facilities, and programs. Some elements of a site HRP include systematic (1) supervisory reviews, (2) medical and psychological assessments, (3) management evaluations, (4) personnel security reviews, and (4) training of HRP staff and critical positions. Over the years of implementing an HRP, the Department of Energy (DOE) has faced various challenges and overcome obstacles. During this 4-day activity, participants will examine programs that mitigate threats to nuclear security and the insider threat to include HRP, Nuclear Security Culture (NSC) Enhancement, and Employee Assistance Programs. The focus will be to develop an understanding of the need for a systematic HRP and to discuss challenges and best practices associated with mitigating the insider threat.

    In this study, we developed and evaluated a brief training program for grant reviewers that aimed to increase inter-rater reliability, rating scale knowledge, and effort to read the grant review criteria. Enhancing reviewer training may improve the reliability and accuracy of research grant proposal scoring and funding recommendations. Seventy-five Public Health professors from U.S. research universities watched the training video we produced and assigned scores to the National Institutes of Health scoring criteria proposal summary descriptions. For both novice and experienced reviewers, the training video increased scoring accuracy (the percentage of scores that reflect the true rating scale values), inter-rater reliability, and the amount of time reading the review criteria compared to the no video condition. The increase in reliability for experienced reviewers is notable because it is commonly assumed that reviewers—especially those with experience—have good understanding of the grant review rating scale. Our findings suggest that both experienced and novice reviewers who had not received the type of training developed in this study may not have appropriate understanding of the definitions and meaning for each value of the rating scale and that experienced reviewers may overestimate their knowledge of the rating scale. Lastly, the results underscore the benefits of and need for specialized peer reviewer training.

    On June 6-10, 2011, the U.S. Department of Energy (DOE), Office of Energy Efficiency and Renewable Energy (EERE), Geothermal Technologies Program (GTP or the Program) conducted its annual program peer review in Bethesda, Maryland. In accordance with the EERE Peer Review Guide, the review provides an independent, expert evaluation of the strategic goals and direction of the program and is a forum for feedback and recommendations on future program planning. The purpose of the review was to evaluate DOE-funded projects for their contribution to the mission and goals of the Program and to assess progress made against stated objectives.

    Over the last 20 years the Idaho National Laboratory (INL) has adopted a number of operations and safety-related programs which has each periodically taken its turn in the limelight. As new programs have come along there has been natural competition for resources, focus and commitment. In the last few years, the INL has made real progress in integrating all these programs and are starting to realize important synergies. Contributing to this integration are both collaborative individuals and an emerging shared vision and goal of the INL fully maturing in its high reliability operations. This goal is so powerful because the concept of high reliability operations (and the resulting organizations) is a masterful amalgam and orchestrator of the best of all the participating programs (i.e. conduct of operations, behavior based safety, human performance, voluntary protection, quality assurance, and integrated safety management). This paper is a brief recounting of the lessons learned, thus far, at the INL in bringing previously competing programs into harmony under the goal (umbrella) of seeking to perform regularly as a high reliability organization. In addition to a brief diagram-illustrated historical review, the authors will share the INLs primary successes (things already effectively stopped or started) and the gaps yet to be bridged.

    Version 00 The U.S. Nuclear Regulatory Commission (NRC) has developed a powerful personal computer (PC) software application for performing probabilistic risk assessments (PRAs), called Systems Analysis Programs for Hands-on Integrated Reliability Evaluations (SAPHIRE) Version 8. Using SAPHIRE 8 on a PC, an analyst can perform a PRA for any complex system, facility, or process. Regarding nuclear power plants, SAPHIRE can be used to model a plant's response to initiating events, quantify associated core damage frequencies,more » and identify important contributors to core damage (Level 1 PRA). It can also be used to evaluate containment failure and release models for severe accident conditions, given that core damage has occurred (Level 2 PRA). It can be used for a PRA assuming that the reactor is at full power, at low power, or at shutdown conditions. Furthermore, it can be used to analyze both internal and external initiating events, and it has special features for transforming models built for internal event analysis to models for external event analysis. It can also be used in a limited manner to quantify risk for release consequences to both the public and the environment (Level 3 PRA). For all of these models, SAPHIRE can evaluate the uncertainty inherent in the probabilistic models. SAPHIRE has evolved with advances in computer technology.« less

    This paper describes the introduction of stochastic linear programming into Operations DER-CAM, a tool used to obtain optimal operating schedules for a given microgrid under local economic and environmental conditions. This application follows previous work on optimal scheduling of a lithium-iron-phosphate battery given the output uncertainty of a 1 MW molten carbonate fuel cell. Both are in the Santa Rita Jail microgrid, located in Dublin, California. This fuel cell has proven unreliable, partially justifying the consideration of storage options. Several stochastic DER-CAM runs are executed to compare different scenarios to values obtained by a deterministic approach. Results indicate that using a stochastic approach provides a conservative yet more lucrative battery schedule. Lower expected energy bills result, given fuel cell outages, in potential savings exceeding 6percent.

The goal of the PARENT is to conduct a confirmatory assessment of the reliability of nondestructive evaluation (NDE) techniques for detecting and sizing primary water stress corrosion cracks (PWSCC) and applying the lessons learned from PINC to a series of round-robin tests. These open and blind round-robin tests will comprise a new set of typical pressure boundary components including dissimilar metal welds (DMWs) and bottom-mounted instrumentation penetrations. Open round-robin tests will engage research and industry teams worldwide to investigate and demonstrate the reliability of emerging NDE techniques to detect and size flaws with a wide range of lengths, depths, orientations, and locations. Blind round-robin tests will utilize various testing organizations, whose inspectors and procedures are certified by the standards for the nuclear industry in their respective countries, to investigate the ability of established NDE techniques to detect and size flaws whose characteristics range from relatively easy to very difficult for detection and sizing. Blind and open round-robin testing started in late 2011 and early 2012, respectively. This paper will present the work scope with reports on progress, NDE methods evaluated, and project timeline for PARENT.

    The Systems Analysis Programs for Hands-on Integrated Reliability Evaluations (SAPHIRE) is a software application developed for performing a complete probabilistic risk assessment (PRA) using a personal computer running the Microsoft Windows? operating system. SAPHIRE is primarily funded by the U.S. Nuclear Regulatory Commission (NRC) and developed by the Idaho National Laboratory (INL). The INL's primary role in this project is that of software developer and tester. Using the SAPHIRE analysis engine and relational database is a complementary program called GEM. GEM has been designed to simplify using existing PRA analysis for activities such as the NRC’s Accident Sequence Precursor program. In this report, the theoretical framework behind GEM-type calculations are discussed in addition to providing guidance and examples for performing evaluations when using the GEM software. As part of this analysis framework, the two types of GEM analysis are outlined, specifically initiating event (where an initiator occurs) and condition (where a component is failed for some length of time) assessments.

    This report summarizes the Accelerated Reliability testing of five lead-acid battery-equipped Chevrolet S-10 electric vehicles by the US Department of Energy's Field Operations Program and the Program's testing partners, Electric Transportation Applications (ETA) and Southern California Edison (SCE). ETA and SCE operated the S-10s with the goal of placing 25,000 miles on each vehicle within 1 year, providing an accelerated life-cycle analysis. The testing was performed according to established and published test procedures. The S-10s' average ranges were highest during summer months; changes in ambient temperature from night to day and from season-to-season impacted range by as much as 10 miles. Drivers also noted that excessive use of power during acceleration also had a dramatic effect on vehicle range. The spirited performance of the S-10s created a great temptation to inexperienced electric vehicle drivers to ''have a good time'' and to fully utilize the S-10's acceleration capability. The price of injudicious use of power is greatly reduced range and a long-term reduction in battery life. The range using full-power accelerations followed by rapid deceleration in city driving has been 20 miles or less.

It can be used for a PRA evaluating a variety of operating conditions, for example, for a nuclear reactor at full power, low power, or at shutdown conditions. Furthermore, SAPHIRE can be used to analyze both internal and external initiating events and has special features for transforming models built for internal event analysis to models for external event analysis. It can also be used in a limited manner to quantify risk in terms of release consequences to both the public and the environment (Level 3 PRA). SAPHIRE includes a separate module called the Graphical Evaluation Module (GEM). GEM provides a highly specialized user interface with SAPHIRE that automates SAPHIRE process steps for evaluating operational events at commercial nuclear power plants. Using GEM, an analyst can estimate the risk associated with operational events in a very efficient and expeditious manner. This reference guide will introduce the SAPHIRE Version 7.0 software.     The overall mission of the U.S. Department of Energy’s Office of Electricity Delivery and Energy Reliability (OE) is to lead national efforts to modernize the electric grid, enhance the security and reliability of the energy infrastructure, and facilitate recovery from disruptions to the energy supply. In support of this mission, OE conducts a portfolio of research and development (R&D) activities to advance technologies to enhance electric power delivery. Multiple benefits are anticipated to result from the deployment of these technologies, including higher quality and more reliable power, energy savings, and lower cost electricity. In addition, OE engages State and local government decision-makers and the private sector to address issues related to the reliability and security of the grid, including responding to national emergencies that affect energy delivery. The OE R&D activities are comprised of four R&D lines: High Temperature Superconductivity (HTS), Visualization and Controls (V&C), Energy Storage and Power Electronics (ES&PE), and Distributed Systems Integration (DSI).

    The Systems Analysis Programs for Hands-on Integrated Reliability Evaluations (SAPHIRE) suite of programs can be used to organize and standardize in an electronic format information from probabilistic risk assessments or individual plant examinations. The Models and Results Database (MAR-D) program of the SAPHIRE suite serves as the repository for probabilistic risk assessment and individual plant examination data and information. This report demonstrates by examples the common electronic and manual methods used to load these types of data. It is not a stand alone document but references documents that contribute information relative to the data loading process. This document provides a more detailed discussion and instructions for using SAPHIRE 5.0 only when enough information on a specific topic is not provided by another available source.

    Through a cooperative effort between the U. S. Department of Energy and the Russian Federation (RF) Ministry of Defense (MOD) a Personnel Reliability Program (PRP) for the nuclear handlers within the RF MOD has been implemented. A key element in the RF MOD PRP is the detection and confirmation of narcotic use in subject military and civilian personnel. This paper describes the process of narcotics screening and testing in the RF MOD and explains the confirmation process once screening has shown a positive result. Issues of laboratory certification, employee certification, employee training, sample chain-of-custody, and equipment needs will be addressed.

    The threat of nuclear terrorism has become a global concern. Many countries continue to make efforts to strengthen nuclear security by enhancing systems of nuclear material protection, control, and accounting (MPC&A). Though MPC&A systems can significantly upgrade nuclear security, they do not eliminate the "human factor." Gen. Eugene Habiger, a former "Assistant Secretary for Safeguards and Security" at the U.S. Department of Energy’s (DOE) nuclear-weapons complex and a former commander of U.S. strategic nuclear forces, has observed that "good security is 20% equipment and 80% people." Although eliminating the "human factor" is not possible, accounting for and mitigating the risk of the insider threat is an essential element in establishing an effective nuclear security culture. This paper will consider the organizational role in mitigating the risk associated with the malicious insider through monitoring and enhancing human reliability and motivation as well as enhancing the nuclear security culture.

    Utility load management programs--including direct load control and interruptible load programs--were employed by utilities in the past as system reliability resources. With electricity industry restructuring, the context for these programs has changed; the market that was once controlled by vertically integrated utilities has become competitive, raising the question: can existing load management programs be modified so that they can effectively participate in competitive energy markets? In the short run, modified and/or improved operation of load management programs may be the most effective form of demand-side response available to the electricity system today. However, in light of recent technological advances in metering, communication, and load control, utility load management programs must be carefully reviewed in order to determine appropriate investments to support this transition. This report investigates the feasibility of and options for modifying an existing utility load management system so that it might provide reliability services (i.e. ancillary services) in the competitive markets that have resulted from electricity industry restructuring. The report is a case study of Southern California Edison's (SCE) load management programs. SCE was chosen because it operates one of the largest load management programs in the country and it operates them within a competitive wholesale electricity market. The report describes a wide range of existing and soon-to-be-available communication, control, and metering technologies that could be used to facilitate the evolution of SCE's load management programs and systems to provision of reliability services. The fundamental finding of this report is that, with modifications, SCE's load management infrastructure could be transitioned to provide critical ancillary services in competitive electricity markets, employing currently or soon-to-be available load control technologies.

    The CONTAIN code was developed by Sandia National Laboratories under the sponsorship of the US Nuclear Regulatory Commission (NRC) to provide integrated analyses of containment phenomena. It is used to predict nuclear reactor containment loads, radiological source terms, and associated physical phenomena for a range of accident conditions encompassing both design-basis and severe accidents. The code`s targeted applications include support for containment-related experimental programs, light water and advanced light water reactor plant analysis, and analytical support for resolution of specific technical issues such as direct containment heating. The NRC decided that a broad technical review of the code should be performed by technical experts to determine its overall technical adequacy. For this purpose, a six-member CONTAIN Peer Review Committee was organized and a peer review as conducted. While the review was in progress, the NRC issued a draft ``Revised Severe Accident Code Strategy`` that incorporated revised design objectives and targeted applications for the CONTAIN code. The committee continued its effort to develop findings relative to the original NRC statement of design objectives and targeted applications.
